What you will need to get started:

• $5 to open your account
• Social Security Number
• Valid Driver’s License, U.S. Passport, U.S. Government ID, or state-issued photo ID
• If the address on your ID isn’t current, proof of address, such as a recent utility bill, cell phone bill, or pay stub

Membership Eligibility

Anyone who lives or works in the following areas of Wisconsin is eligible for Unison Membership: Brown, Calumet, Manitowoc, Outagamie, or Winnebago. Additionally, regardless of where you live or work, if you are an immediate family member of a current Unison member, you’re eligible.
